About an inch inside your rectum, the skin of your butt ends, and the lining of your colon begins. While the skin area has many nerve endings and can be very sensitive to pain, the colon lining area is insensitive to pain.

Internal hemorrhoids form about one to two inches inside the rectum in the painless, colon lining region. But when they become inflamed, the entire region can become inflamed, including the more sensitive skin area. That’s how internal hemorrhoids can cause pain externally.

Because the pain can be so significant, many people mistakenly believe that treatment will be painful as well. Or they’ve heard horror stories about surgery and are afraid the pain will be worse.
